To be eligible for a scholarship, applicants must live in one of the identified eleven counties and MUST have a medical diagnosis before applying.
Scholarship Guidelines must be read thoroughly prior to completing the application. Applications that do not follow the Scholarship Guidelines will not be considered for funding.
Spring Cycle: applications may be submitted beginning January 1 and are due no later than 11:59pm on February 1. All applicants will then be notified the week of March 1 of their scholarship status.
Fall Cycle: applications may be submitted beginning July 1 and are due no later than 11:59pm on August 1. All applicants will then be notified the week of September 1 of their scholarship status.
Applications submitted before the start of a scholarship cycle or after the deadline will not be accepted.

We require an explanation of benefits, a screenshot of what your insurance does and does not cover, or a denial letter.
If you have previously received a Family Hope Foundation scholarship, you are still required to submit all of the documentation for the current scholarship.

You or the individual you are applying for MUST have a medical diagnosis in order to qualify for a scholarship. The diagnosis must come from a licensed doctor, physician, or clinical psychologist / psychiatrist.
This documentation could be an evaluation summary listing the diagnosis, a letter from a doctor, a medical visit summary document containing the diagnosis, or a medical issue list sometimes compiled in certain patient portals through doctors offices.
*Please note, if your evaluation was from a SLP, the diagnosis must be signed off by your doctor or physician in order to qualify for a scholarship.
We will not accept an IEP, a letter or statement from a therapist (unless the therapist is the licensed diagnosing clinical psychologist or psychiatrist), a letter from school, a letter from a nurse or home aid, or a letter from a school psychologist. The diagnosis must come from a clinical doctor, physician, or psychiatrist.

Our therapy scholarships are to be utilized for any future therapy appointments at your designated therapy provider center. Family Hope Foundation scholarship funds cannot be used for past-due balances, respite-type activities, evaluations, missed appointment fees, or medical procedures.
