Applicants must live in one of the identified eleven counties.
Scholarship Guidelines MUST be read thoroughly prior to completing the application. Applications that do not follow the Scholarship Guidelines will not be considered for funding.
Spring Cycle: applications may be submitted beginning January 1 and are due no later than 5pm on February 1. All applicants will then be notified on March 1 of their scholarship status.
Fall Cycle: applications may be submitted beginning July 1 and are due no later than 5pm on August 1. All applicants will then be notified on September 1 of their scholarship status.
Applications submitted before the start of a scholarship cycle or after the deadline will not be accepted.

For our Spring Cycle, applicants will be notified of their award status on or by March 1st. For our Fall Cycle, applicants will be notified of their award status on or by September 1st.
We require an explanation of benefits, a screenshot of what your insurance does and does not cover, or a denial letter.
If you have previously received a Family Hope Foundation scholarship, you are still required to submit all of the documentation for the current scholarship.

If you receive a scholarship, a scholarship committee member will contact you within a few days of receiving your scholarship award letter. Please correspond with your designated scholarship committee member with all of your questions.
At this time, Family Hope Foundation is not accepting new provider requests. We are continuously working hard to operationalize our scholarship process. Our community will be notified when we are accepting new provider requests again.
Reminder: Family Hope Foundation scholarship funds cannot be used for past-due balances, respite-type activities, evaluations, missed appointment fees, or medical procedures.
